  • We know it's often hard to figure out how to determine what type of care you need, but now it's easier than ever to determine the care that best suits your needs with our knowledgeable staff.

What to Know Before Your Appointment

  • Make sure to bring your current Insurance cards, any copayments, and current medication list for each appointment.
  • We do not take Chronic Pain or Anxiety Patients. You may receive a referral from us to receive treatment for these conditions.
  • We do not do workman's compensation.
  • We do offer a sliding fee scale for discounted services for those in need, see information below.

Sliding Fee Scale

  • This practice serves all patients regardless of ability to pay.
  • Lawrence County Health Department will offer a Sliding Fee Discount Program to all who are unable to pay for their services.
  • Discounts for essential services are offered based on family size and income.
  • Lawrence County Health Department will base program eligibility on a person’s ability to pay and will not discriminate on the basis of an individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.
