What to Know Before Your Appointment

  • Make sure to bring your current insurance cards, copayments and current medication list for each appointment.
  • We do not take chronic pain or anxiety patients. You may receive a referral from us to receive treatment for these conditions.

Sliding Fee Scale

  • This practice serves all patients regardless of ability to pay.
  • Lawrence County Health Department will offer a Sliding Fee Discount Program to all who are unable to pay for their services.
  • Discounts for essential services are offered based on family size and income.
  • Lawrence County Health Department will base program eligibility on a person’s ability to pay and will not discriminate on the basis of an individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.
